Privacy Notice

Personal Information I will collect:

  • Name

  • Gender (or preferred identity

  • Age

  • Date of Birth

  • Relationships & Children

  • Occupation

  • Address

  • Telephone/SMS number (plus permission to send SMS & leave voice message)

  • Email address

  • Counselling/Mental health support History

  • Medical conditions relevant to counselling

  • Prescribed medication relevant to counselling

  • Difficulties

  • Session summary

 

How I will store your personal information:

  • Paper: written notes kept in a locked filing cabinet, in either a locked office or my home address.

  • Non-Smartphone: I use a non-smartphone and don't store contacts and regularly delete texts and call logs. .

  • Email/SMS: your email address and correspondence will be stored in my email account by nature of you contacting me. Your telephone number may be stored in my SMS should we exchange messages this way. All correspondence stored there will be deleted regularly and on our work ending together.

  • Website: none of your personal information is stored on my website which is hosted by Wix, other than to momentarily collect & send it to my email account if you use the contact form for initial communication. Wix and Google analytics collect non personal data without your identity attached such as traffic and number of page visits (cookies) to help me evaluate my website.

How I process or share your personal information:

I will not share your personal information except for in the following circumstances:

 

Emergencies

  • If I am concerned for your wellbeing, I may share your contact information with an emergency healthcare service (e.g. Mental Health Crisis Team).

  • If I have become aware of your intent to cause harm to another person/organisation (e.g. terrorism), or commit a serious crime, the law may require that I inform an authority without seeking your permission.

  • Should you be in active therapy and I become incapacitated, or in the event of my death, your information will be shared with my therapeutic executor who will notify you

 

Deleting your Information

  • When we finish working together, I will delete any emails/SMS within one month

  • I may hold your clinical information for up to six years after you finish therapy with me. This is a requirement of my insurance and also allows me to reflect on past work if you return to counselling at a later date.

 

After this time your information will be securely destroyed.

 

Your rights under GDPR

You have the right:

  • To be informed what information I hold.

  • To see the information I hold about you.

  • To rectify any inaccurate or incomplete personal information.

  • To withdraw consent to me using your personal information.

  • To request your personal information be erased (I may decline this request if the information is required for legal or insurance purposes).
